I hereby authorize the directors of any Showcase or Camp taking place at Fort Scott Community College to act for me according to their best judgment in any emergency requiring medical attention. I know of no mental or physical problems that may affect my son's ability to safely participate in any Showcase or Camp taking place at Fort Scott Community College. I hereby waive and release the coaches/staff of Fort Scott Community College from any liability for illness or injuries why participating in any Showcase or Camp taking place at Fort Scott Community College.
